Maryland Coalition to Control Diabetes (MCCD)
The Diabetes Prevention and Control Program (DPCP) has been fortunate to work with a wide variety of organizations and agencies that have been involved in planning and implementing activities to prevent and or control diabetes. In 2004, a Coalition of approximately 30 organizations was formed. The Coalition created a strategic plan to reduce the burden of diabetes in Maryland. The resultant document, the Maryland Diabetes Prevention and Control Plan is a blueprint for preventing diabetes and its complications.
The Coalition meets quarterly as needed. Membership in the Coalition is open to organizations or individuals who share the mission, vision, and goals of the Coalition. In addition, the Coalition serves as an advisory group to provide advice and guidance for the DPCP work plan.
Mission of the Coalition
The mission of the Maryland Coalition to Control Diabetes (CCD)is to reduce the burden of diabetes on the residents of Maryland by providing information, technical assistance and support to organizations and agencies as they plan, implement, and evaluate their activities directed at preventing or delaying the onset of diabetes and its complications.
Vision of the Coalition
The Maryland CCD envisions a future where all Maryland residents are aware of the risk factors for diabetes, and have adopted healthy behaviors that minimize their risk of developing diabetes. For those with diagnosed diabetes, we envision full access to recommended care, fewer people with complications of diabetes, and empowered people with diabetes that can self-manage their disease.
Goals of the Coalition
The Maryland CDD will:
- Serve as an ongoing statewide forum to identify and address issues critical to the success of Maryland’s collective efforts to prevent the onset of diabetes and to prevent complications in those with the disease;
- Become the liaison between organizations and agencies within the State to promote linkages and partnerships that will better maximize the use of resources while promoting best practices;
- Provide expertise and coordination in the effort to collect, analyze, and disseminate data that defines the burden of diabetes in Maryland as well as collect evidence of progress in the prevention of diabetes and its complications; and
- Advocate for health policy changes and/or legislation to generate resources that will be necessary to ensure the success of Maryland’s collective efforts to prevent the onset of diabetes, and to prevent complications in those with the disease.
