What is Care Coordination?

Care coordination is a process that:

  • Engages children and youth with special health care needs (CYSHCN) and their families in the development of a care plan;
  • Links them to health and other services that address their needs and concerns; and
  • Works to promote optimum health, functioning, and quality of life for CYSHCN and their families

The Office for Genetics and Children with Special Health Care Needs supports care coordination for CYSHCN in a number of Local Health Departments free of charge. The services available vary by county. Please click on the link below to find out what is available in your county.
